What is it used for |

PSEA Standing Order Form
|
The PSEA scheme helps parents save for their children’s post-secondary education by maintaining a Post-Secondary Education Account (PSEA) for each eligible child. This is part of the Government’s efforts to encourage every Singaporean to complete his post-secondary education. It also underscores the Government’s commitment to support families in investing in the future education of their children, and to prepare them for the economy of the future. PSEA is not a bank account. It is administered by the Ministry of Education and is opened automatically for all eligible Singaporeans.

Eligible student can withdraw the PSEA funds to pay for school fees by establishing a Standing Order with MOE. The Standing Order (SO) is a one-time application and the SO instructions will apply to all future withdrawals until the SO is terminated or replaced, regardless of which approved institution you attend.
Please complete the PSEA Standing Order Form and drop the completed form at the PSEA Dropbox located at the ITE College East Customer & Visitor Centre.

PSEA Ad Hoc Withdrawal Form
|
The ad hoc withdrawal application is an application for a withdrawal for a specific purpose. You will have to submit the ad hoc withdrawal application forms for each specific withdrawal.

PSEA Standing Order Termination Form
|
If you wish to terminate your current Standing Order instruction with MOE, please complete the PSEA Standing Order Termination Form and drop the completed form at the PSEA Dropbox located at the ITE College East Customer & Visitor Centre. |

Student Leave Form
|
If you require to take time or day off from your class, you will need to submit the Student Leave Form with supporting document to Class Advisor prior to your absence or immediately when you return to class. |
| CCA Leave Application Form |
If you require to take time or day off from your class to take part in your CCA activities, you will need to submit the CCA Leave Application Form, with endorsement by your CCA teacher-in-chargeb to Class Advisor prior to your absence or immediately when you return to class. |
| Application_for_Certified Statement / Statement of Results / Academic Transcript |
Please complete this form if you require the following types of Statement:
- Statement of Results – shows module results attained by the applicant in one examination series.
- Academic Transcript – shows all module results attained by the applicant in the programme.
- Certified Statement – certifies award of certificate. It is issued in place of the original certificate.
- Certified Statement for Full-time Higher Nitec or Nitec Early Leavers – shows module(s) passed by full-time applicants who left without completing the programme of study.
A non-refundable administration fee of $10 is payable for each type of statement(s) requested within the same application form.
You may mail the application by post to "ITE HQ Customer Service Centre at 2 Ang Mo Kio Drive, Singapore 567720" accompanied by a crossed cheque made payable to Institute of Technical Education.
Alternatively, you may submit your application form to ITE College East Customer & Visitor Centre with cash payment.

| Authorisation Form For Certificate Collection |
Students may collect their Certificates and Academic Transcripts from the Customer & Visitor Centre of their Campus of Study. Please note that Certificates will be kept at Colleges for 2 years from the date of issue of certificates and uncollected certificates after 2 years are to be returned to CEXM Dept for disposal at the end of 2-year retention period.
If you are not able to collect your Certificate, you may authorise someone to collect your Certificate on your behalf by using the Authorisation Form For Certificate Collection. Students The authorized person must present the completed form and his/her NRIC/passport as proof of identity when collecting the Certificate.

| GIRO Form (For Inter-Bank GIRO School Fees) |
Students may choose to pay school fees through Inter-Bank GIRO. Please complete the form and submit the form to the ITE College East Customer & Visitor Centre. The GIRO arrangement will be for the entire course of study. |
| GIRO Form (For Direct Credit Authorisation) |
Students may take on work on a part-time basis in ITE Colleges or at selected external companies under the Realwork @ College Scheme. Students will receive an allowance based on the rate agreed upon when they commence work. Students must apply for GIRO arrangement with ITE before they commence work. To do so, they may submit the completed form to ITE College East Customer & Visitor Centre.

| Application for Ezlink Card Extension |
ITE full-time students who are re-admitted and attending class on a regular basis are eligible for extension of their existing ITE ez-link cards. Repeaters who are only sitting for exam and not attending class on a regular basis are not eligible for extension of ez-link card. |
| Declaration for Above 25 years old |
ITE full-time students who are above 25 years of age need to submit a declaration form to TransitLink in order to possess a EZ-Link card with Student Concession. |

Declaration of Household Income Form
